I have an instrument track recorded, and on the HALionONE I've turned all effects off, yet when I play back I have a very notable reverb of unknown source.
What should I be doing to turn ALL effects off?

I think I recall that Haion1 has an internal reverb. I know I tweak it sometimes when I'm using some of the Halion1 patches (there's also chorus and other effects that might be controllable from the Halion instrument itself).

If it is the built in reverb, then I think you can display the Halion1 vst instrument screen and there would be a "Reverb" knob. That's the way I've seen it on my system.
